Forms

Once you make an appointment with Dr. Butterfield, you will be e-mailed an intake packet to fill out online prior to your appointment.  Please be sure to fill out the forms and sign the treatment agreement.  If these forms are not completed before the appointment, they will need to be done during the appointment, which will limit the time available for the assessment.
